Financial Policy

Our fees reflect our services. However, insurance companies only pay us a percentage of our charges. Most times, our contract with your insurance company requires the difference to be adjusted off and you are not left with a balance. Sometimes, your contract with your insurance company requires that the balance is the responsibility of the patient. In all cases where there are pre-determined co-payments, your insurance company requires that these be paid at the time of service. Your insurance company asks that we advise them of patients who are not complying with this policy. If it is necessary for us to send a bill for your co-payment, there will be a $5.00 processing fee. If we will not be billing insurance for your services, are not participating providers with your insurance, or if a non-covered service is provided, payment is due at the time of service.

It is important that a fully completed, current patient registration is on file at all times to ensure that appropriate contact numbers are available to us and appropriate details are on file for billing insurance. Our registration also includes a release of information section that allows us to file claims and give information to your insurance company so you are not inconvenienced with this tedious process. A governmental regulation limits this release to one year.

We know that appointments can be forgotten and emergencies arise. That is why we allow three missed appointments before a patient is asked to leave the practice. When you miss an appointment without giving 24-hour notice, you are denying another patient the opportunity to be seen in your place. We charge $25.00 for missed appointments and $50.00 for missed annual physical exams without a 24-hour notification.

All medical record requests are outsourced to Smart Corporation Copy Service. If you require us to transfer your records to another primary care physician, Smart Corp may bill you for this service. Governmental regulations limit, but allow for, these fees and require us to obtain a Medical Records Release Authorization form prior to the release.

Your health insurance company requires that your car insurance or the driver at fault pay Motor Vehicle accident services. While Community Primary Care does not bill motor vehicle insurance, we are happy to provide you with any forms or information required for your reimbursement. Payment in this situation is required from the patient at the time of service.

We are happy to fill out paperwork for FMLA, Disability, etc. However, the administrative costs must be offset by a nominal processing fee. Therefore, a processing fee of $15.00 is payable prior to completion for FMLA forms, disability forms and other similar forms.

If at 90 days past due, we are unable to collect the patient portion of an account, the account will be placed with a collection agency or attorney. In that event, the person responsible for the account will be responsible for all collection costs including reasonable attorney fees and court costs. At this time, the patient physician relationship will be terminated and the patient will be required to obtain a new primary care physician within 30 days.

Physicians do not discuss financial issues. Our billing staff member is trained to discuss your account and make acceptable payment arrangements. They will be happy to help you; however, if you need further assistance, our billing manager can be consulted as well.
